June 2026 Research Update

This report covers research on Waldenstrom macroglobulinemia published in PubMed in June 2026. For other months, see all research updates.

Last update: July 2, 2026

At a glance

Post-BTK treatment
Bortezomib-based treatment often worked after BTK inhibitors.
First-line treatment
Obinutuzumab plus bendamustine had high response rates as first treatment.
Free light chain tests
Two free light chain blood tests mostly agreed overall.
Treatment and testing
This month added evidence for treatment options beyond BTK inhibitors and for more consistent monitoring tests. Side effects, especially infections and low blood counts, remained a key issue.

Overview

This month, the clearest changes were in treatment options beyond BTK inhibitors, plus new work on how WM is measured.

Two treatment reports added evidence that WM can still respond in different situations. Bortezomib-based treatment had responses in many people who had already received BTK inhibitors. Severe nerve damage was not common in that small series. Another study reported high response rates with obinutuzumab plus bendamustine as a first treatment in a group that included WM.

Side effects were a steady theme alongside those responses. Low blood counts and infections were prominent in these reports. The bortezomib series reported some serious low neutrophil or platelet counts.

On the testing side, two lab-focused studies aimed to make monitoring more consistent. One comparison found close agreement between two blood tests that measure free light chains, with most mismatches happening near the cutoff between normal and abnormal. Another test measured common WM mutations, and the mutation signal tended to be higher in symptomatic WM. In samples followed during ibrutinib treatment, mutation levels fell in people described as responders, although the signal in cell-free DNA could be very low.

Limitations

  • Treatment results came mainly from non-randomized studies without comparison groups.
  • Some treatment evidence was based on a small single-center series with varied regimens.
  • Testing advances were lab-focused and not shown to change care decisions yet.
  • Several complication findings were single-patient case reports that cannot show how often they occur.
