Overview
Caplacizumab results for immune TTP looked mostly consistent this month, with quick platelet recovery in routine care and delays more often tied to other medical causes than to true drug failure. The evidence leaned heavily on real-world registry data, safety reporting systems, and a small number of detailed case reports, so the themes were practical but not always able to answer how often something happens.
A repeating pattern across treatment and monitoring was that "not improving as expected" can have more than one explanation. In a registry covering 204 acute immune TTP episodes, platelet counts commonly recovered within the first few days, and the uncommon situations labeled as refractory often had alternative reasons for ongoing low platelets. Alongside that, post-marketing reports continued to point to bleeding as the most frequent type of safety concern, with many reports describing onset early after starting caplacizumab.
Diagnosis and outcomes work stayed focused on real-world complexity. A national hospital database analysis reported no adjusted mortality difference for weekend versus weekday TTP admissions, but it did link later start of plasma exchange and smaller hospital size with higher in-hospital death. Case reports added concrete examples of why teams may revisit early assumptions, including malignant hypertension mimicking TTP and an initially negative ADAMTS13 inhibitor test despite very low ADAMTS13 activity.
More detailed questions about rare resistance, drug triggers, and organ involvement are still evolving in mostly case-based evidence.

Limitations
- Post-marketing safety reports are voluntary and cannot estimate how often bleeding occurs or prove causality.
- Several key signals came from single-patient case reports, including suspected caplacizumab resistance and drug-associated TTP.
- The hospital outcomes findings were based on administrative hospital data and measured in-hospital outcomes only.
- Mechanism and genetics discussions mixed reviews, pharmacovigilance signals, and limited clinical confirmation across settings.
