September 2026 Research Update

This report covers research on Metachromatic leukodystrophy published in PubMed in September 2026. For other months, see all research updates.

Last update: October 10, 2026

At a glance

Transplant timing
Earlier donor stem cell transplant was linked to better daily function.
Ongoing symptoms
Transplant did not always stop symptoms from getting worse.
Gallbladder changes
Gallbladder changes were often reported across different MLD stages.
Timing and whole-body issues
Long-term data linked HSCT benefit to earlier timing, especially before symptoms. Reports also highlighted non-brain issues, including gallbladder changes.

Overview

This month, the clearest updates were about donor stem cell transplant timing in late-onset metachromatic leukodystrophy (MLD), plus broader health issues outside the brain and nerves.

Long-term registry data added support that allogeneic hematopoietic stem cell transplantation (HSCT, a donor stem cell transplant) can slow progression in late-onset MLD. The strongest pattern was seen when HSCT happened before symptoms started. Quality-of-life and day-to-day functioning were reported as better with HSCT than with no HSCT. The same long-term data also showed that HSCT did not always prevent symptoms from getting worse.

A separate set of reports highlighted non-neurologic health issues. A systematic review found gallbladder abnormalities were frequently described in published MLD reports. These gallbladder findings were reported across the MLD spectrum. They were described both before neurologic diagnosis and after disease-modifying treatment in some reports.

Finally, a pediatric inpatient database study across many leukodystrophies added context about severe hospital illness. Overall inpatient mortality was reported as 5.2%. MLD made up 28% of recorded inpatient deaths in that dataset. Deaths were more common in the setting of intensive care, infections, and mechanical ventilation.

Limitations

  • The HSCT findings are from registry data, so outcomes may be influenced by who received transplant and when.
  • The gallbladder review reflects what was reported in published cases, so it cannot show how common problems are in all people with MLD.
  • The inpatient mortality results came from a mixed leukodystrophy dataset and do not explain exact causes of death for MLD.
