Langerhans Cell Histiocytosis
New Langerhans cell histiocytosis research, summarized each month in plain language.
2026 so far: So far in 2026 (January-September), real-world data added detail on MAPK inhibitors in refractory LCH. Many reports described rapid control, but there were gaps for lung/CNS disease, relapse after stopping, and frequent side effects. Read the 2026 review

Yearly update
2026 so far
- MAPK inhibitor results. Evidence broadened from refractory control to CNS-specific cohorts.
- After relapse options. More salvage approaches reported, including phase II clofarabine.
- Complications and support. Sharper risk separation for liver disease; new bone-support data.
- Watching single bone lesions. Large cohort clarified who often stays stable without treatment.
- Staging and biomarkers. More PET/CT staging data; early neurodegeneration markers explored.
New
September 2026
- Adult routine care. Routine-care data described common treatments and reported responses.
- Bone health measures. Vitamin D use was linked with lower bone breakdown markers.
- Relapse risk factors. In children, high-risk organ involvement best predicted later relapse.
- Neurodegeneration tests. Early data linked spinal-fluid IL-1RT2 with neurodegenerative LCH.
August 2026
- Adult first treatment. Cytarabine was often the first systemic treatment in adults.
- Targeted drugs. Some relapsed adults responded to targeted drugs matched to mutations.
- Diabetes insipidus risk. Some organ patterns were linked to higher diabetes insipidus risk.
- Thinking symptoms. Thinking problems could persist even when brain scans improved.
July 2026
- Chemo with maintenance. One small center reported long-lasting control after chemo plus maintenance.
- Relapse treatments. A small series reported remission after several different salvage treatments.
- Scans and biopsy. Scans and biopsy were often both needed to confirm disease spread.
- PET/MRI vs PET/CT. PET/MRI sometimes added detail, while PET/CT better assessed lungs.
June 2026
- MAPK-targeted drugs. Symptoms and scans often improved, but side effects were common.
- Liver function risk. Liver dysfunction was linked with lower survival than enlargement alone.
- FDG PET/CT staging. Whole-body FDG PET/CT often found more sites and changed staging.
May 2026
- Confirming LCH. Imaging helped map where lesions were, but a biopsy with specific lab stains was used to confirm LCH.
- Pituitary MRI over time. In pituitary stalk thickening, repeat MRI over time helped guide when a biopsy was considered.
- Early monitoring tools. Blood-based BRAF tracking and computer analysis of MRI were explored, but these tools are not yet proven for routine use.
- Clofarabine tradeoffs. In relapsed or treatment-resistant pediatric LCH, clofarabine often reduced disease, but serious side effects were frequent.
April 2026
- Careful follow-up without treatment. In several reports, many children with a single bone LCH lesion stayed stable for years with watchful waiting and regular follow-up.
- Changes often show up in year one. When new events occurred, they most often appeared within the first year after diagnosis.
- Imaging finds sites; biopsy confirms. Scans helped map possible disease sites, but a tissue biopsy was still needed to confirm LCH because other conditions can look similar on imaging.
- Blood markers for neurodegenerative LCH. Neurofilament light chain was higher in neurodegenerative LCH, but testing at diagnosis had limited ability to predict who would later develop neurodegeneration.
March 2026
- Treatment after relapse. Several small studies reported disease control in relapsed or refractory LCH during newer treatments.
- After stopping targeted therapy. Some children relapsed after targeted medicines were stopped, even if the disease had been controlled during treatment.
- LCH in uncommon sites. Reports described LCH presenting in less expected locations, including infant skin and the stomach.
- Bone relapse risk signals. In children with bone-involving LCH, more bone sites and a higher starting white blood cell count were linked to more follow-up events.
February 2026
- MAPK inhibitors in children. In children with refractory LCH, off-label MAPK inhibitors were linked with rapid improvement in many patients, but responses were less consistent for lung destruction and nervous system-related disease.
- Lung LCH risk markers. In adults with lung LCH, higher TNF-alpha and MMP-7 at diagnosis were seen in people whose breathing later declined, but this is early prognostic research and not a routine test.
- Emotional and behavior concerns. Questionnaire studies found that some children had emotional or behavioral concerns, and quality-of-life scores varied across family and treatment-related factors.
- Biopsy markers to separate look-alikes. In one pathology series, CD71 staining appeared in Rosai-Dorfman disease samples but not in LCH, and case reports reinforced that biopsy markers can be essential when LCH mimics infection or cancer.
January 2026
- Cladribine and lung function. In a small trial of adults with pulmonary LCH, cladribine was followed by better breathing tests and symptom scores, focusing attention on measurable lung changes rather than scans alone.
- Pulmonary hypertension treatment. In three people with pulmonary hypertension related to pulmonary LCH, adding sotatercept was followed by large improvements, and side effects such as low platelets and bleeding symptoms were reported.
- Long-term outcomes in children. Pediatric follow-up studies reported high survival but ongoing relapse and later complications, and one large series linked uncommon hearing loss to later brain-related problems.
- Where diagnosis can go wrong. Reports noted that LCH can look like other bone conditions on imaging and can be missed on small lung biopsies, so broader imaging and confirmatory testing may be needed to separate look-alikes.
