Overview
This month was mainly about what treatment and follow-up can look like across different forms of Castleman disease. The clearest theme was that the subtype and how widespread the disease is often shaped what happened next.
For unicentric Castleman disease (one main area), reports continued to support local treatment as a common path. A small hospital series described no recurrence after complete surgical removal during follow-up. One unresectable mediastinal case improved after radiotherapy, with tumor shrinkage and better breathing.
For multicentric Castleman disease (multiple areas) and related inflammatory syndromes, the literature showed a wider range of courses. Several different systemic drug combinations were used across small reports. Some single-patient reports described remission or response, including in HHV-8-associated disease where the HHV-8 viral load became undetectable after treatment. One report described a severe TAFRO syndrome course that stayed difficult to control despite multiple immune-targeting treatments.
Case reports also broadened the picture of how Castleman disease can show up and how it is tracked. Presentations included pleural effusion, nerve problems, and overlapping syndromes like POEMS-like features or paraneoplastic pemphigus. One separate case described two different PET/CT tracers visualizing involved lymph nodes, as early context rather than proof that one scan improves outcomes.

Research topics this month
Limitations
- Much of the evidence came from small retrospective series and single-patient reports.
- Treatments differed widely across patients, so comparisons between options were not tested.
- Imaging findings were described in one person and did not show outcome benefit.
